What you can do to "defragment" things is to rebuild (NOT re-organize) all the indexes on the various tables. Keep in mind, though, that this process will likely cause both...

My employer requires smart cards for login, and it works fine.
You should be able to add your AD login as a login in SQL, then just start SSMS...
