Except for the occasional hard-disk problem, performance problems usually exist (there are always exceptions to every rule) in the code.

For data dictionary information, I simply store extended properties with the table. When you send the code or the database to someone, the documentation is auto-magically included.

It's funny... SSMS shows "INS" and "OVR". It wouldn't have killed them to add "CAP" to the status bar.
