A table of all possible combinations? Is that feasible? If you have 10 diagnoses, you have over 3-million possible combinations, and the numbers go up fast from there.

The speed really depends on how you set it up.
I currently have a table with over 30-million rows of varbinary(max) data in it. I'm in the process of partitioning...

I would definitely consider breaking that into its own partition.
As for performance, et al, the only way to be sure is to test it thoroughly.

I'd say the thing to do is look for companies hiring a junior DBA position. Trying to get a senior position in order to learn senior skills is backwards....
