Misspelling names, I could probably get a former co-worker to provide a list of at least 14 ways Canada can be misspelled and it only uses 4 unique letters.

HIPAA actually has very little to do with encryption on the DB side. It's more with tracking who is accessing what, and whether they have permissions....
