    That's essentially either two different reports with different queries, or one complex query that uses that parameter to determine which of two Select statements to use.

    I'd go with two reports....

    Are the dates the column headers?

    If so, you should be able to query the table using OpenRowset, and then pivot it in T-SQL, instead of in an SSIS connection.

    You can use DDL triggers to fire on creation of a procedure. They can check values in tables. Will that do what you need?

    Definitely split it out into a sub-table. That'll make it a lot easier to work with, and it will be faster to update, query, etc.
