Looks like the login account does not have enough permissions (probably "inserts into some table" in trigger).
Where do you see a permissions related error?

The create table is the cause. That takes schema mod locks and those block absolutely everything.

Yup, add a new nullable column, update in small batches running log backups between the batches to prevent the log from blowing out (that's probably what used all the space).

I deleted my Maintenance Plan and only selected the databases that I created and ignore Master, and model for the time being.

Look in the SQL error log. Sounds like someone's been fiddling with things they don't understand. Look for any and all messages relating to model.
