Just out of curiosity, anyone ever actually created (or seen) a server with 50 instances of SQL installed on it? Most I've ever done is 6.

We had a simlar thing happening several months ago on several non-clustered SQL servers after upgrading to SQL 2005.
We ended up finally tracking it down to a 3rd...

Oh, yea, if you have the DB attached somewhere you can just query out of the system tables.
