Not necessarily. A table with a lot of pages typically indicates that you have a lot of data in that table.

Could you provide a table definition along with the columns that need to be returned in the query (rather than just *)?

For me it depends is a good answer. I believe tables should have a clustered index (much the same reasons as Gail). I do not always create it...

I tested this on SQL 2008. I got exactly the same behavior as the OP.Maybe there is a difference here between 2005 and 2008 ?

As Gail has said, there was a bug related to this. Please verify that both servers are running the same service pack level - update to sp3 if applicable....
