You say there are no bad password attempts from the application, but are there bad attempts coming from anywhere else? Maybe somebody else is trying to guess that password.

Try to create a local logon to each of the servers and give it the same password (same account same password). Then use that account to run the services...

You should be able to run TCP/IP alongside IPX. IPX is not a protocol supported by SQL Server. You should probably use SQL logins instead of windows auth...

What is your service pack? Sounds like maybe a connection pooling issue that was fixed by service packs.
