Has anyone seen this msg before.
Error: 823, Severity: 24, State: 3 followed by
I/O error 21(The device is not ready.) detected during write at offset 0x000000238b0000 in file 'XXX..'. any idea when this happens and whats the solution. is it a h/w failure or a database related problem?

You may want to check what you were doing when this error occured. If you were writing backups the verify the backup drive exists and is ok. Can you read/write files on this drive? It is likely a hardware fault of sorts.

I backup to disk and then copy those files to tape. Sometimes I have seen the error you are getting. When it happens, it's usually because my 'copy to tape' job is using the backup file and SQL Server can't backup to it as that would mess up the copy to tape.
