When people are scared to make a decision, is it because an organisation has a blame culture?
The culture needs to support the principle that we make the best decision we can with the information and time available. If facts emerge that reveal a better decision is available, we acknowledge that our previous decision was made in good faith and was the best for the constraints we faced. New facts indicate that new decisions may be required.

My current company's IT department suffers from analysis paralysis. We spend WAY too much time trying to make perfect decisions or processes and it stops us from getting anything accomplished. I can understand why given the background of the IT team, but it is frustrating to work with. I thought some people were lazy when I first started here, but now I realize they just spend time inefficiently due to this mindset.

One observation I have from across my career is that people agonise over which product is best rather than which one meets their needs for the next 3-5 years. The "best" may offer loads of features that have little relevance to your organisation.
