Issue Code Placeholder message window not going away

I am now getting this Issue Code window that started showing up and I can't seem to get rid of it. When I restart Mssql, it keeps coming up.
What is it and how do I get rid of it?

This looks like an add-on, perhaps SQL Prompt or something similar.
From your picture it is hard to know what is going on, but it looks like you could disable that rule. -
That could be.
But if I press the disable rule, I get an error and there is no close button.
