Back in my early days, many moons ago even before databases were common, I wrote a script which deleted a data entry file and renamed the backup file. You can't run that script twice. My operators had to redo a couple hours of data entry.

Back in 2003 or so our production databases started disappearing one day. We called the DBA and his response was "I'm testing a new migration tool. It has a flag to delete the database when the migration is complete. I'll get the databases back in a little while". Geez, dude, you didn't read the damn instructions? Why were you testing this tool against a production server? Fortunately it was not a busy time of the month and we did not lose any data.
