Take a look at "Create Event Notification" and the linked articles from it, in Books Online. Should have the data you need in there.

The CDs/DVDs are just a convenient way to install on a server that doesn't have an Internet connection, or to reinstall without having to redownload.

Second the motion on "SQL Server 2005 Bible". Excellent book. The 2000 version of that book made it possible for me to learn SQL when I needed to.

As always, I'm going to recommend doing pivot operations outside the database. If you absolutely MUST do them in the database, check out the data here:

This, "SUBSTRING(TPSAptfor,1,CHARINDEX(',',TPSAptfor,1)-1)" will give you an invalid substring length error (which is the one you seem to be getting) if there is no comma in the field.

I didn't realize your top level had a parent ID equal to its Individual ID. You'll need to add a test for that to the Where clause.
