If you have an admin login, you can reset the sa password to whatever you like. There's no "getting it back".

Usually, in the folder for the page/site you selected. That'll be wherever you have IIS publishing from (or whatever you're using for the report server to serve pages).

You can create a table that doesn't have a Primary Key constraint, but if it doesn't actually have any set of columns that uniquely identify the rows, then...
