I'd go with Jack's design. Don't bother with denormalizing the expiration or last payment date into the users table. Just query the payments table to get that.

You can bypass the table variable step if you specify the columns in Tbl5 in the Output clause.

I'd say building something in Access (or .NET, even better) is probably your best option.
Most databases are designed to be accessed via a specific application, set of applications, or SAS...
