GAM pages are per file. Each file will have its own GAM pages tracking extent allocation within that file.

Take the database into single user mode and run CheckDB with the repair_rebuild option.

Since you're on SQL 2008, take a look at Change Tracking, Change Data Capture and SQL Audit. I'm sure one of them will serve your purposes.

The simplest way would be to ensure that no windows accounts (including Builtin\administrators) are granted logon to the server. Either disable, revoke connect or drop them completely.

Useful indexes will reduce IO load, not increase it. Trick is to find useful indexes.
