What defines that a row is 'first'?SQL doesn't have any internal insert time or anything like that and tables have no order.

Thanks for the info Gail. The default log file path was incorrect. Its now upgraded to R2.

If you want one file per backup, the maintenance plan will do that, though most people recommend not generally using the maint plan.
This set of options will create one file...

SQL has no such limitations or errors. If you ask it for a million rows, it'll send you a million rows. Has to be something, some setting, in the front...

In 2005, the TOP (100) PERCENT ... ORDER BY is completely and totally ignored. The optimiser knows that the top won't limit rows and it will no honour any order...

Open up the event log or SQL Agent log and have a look at the messages in there. Should tell you why the start failed.
