OK, the reason I ask is I was just wondering if you've considered moving to another location, possibly with more opportunity?  There are a lot of places hiring out here (NYC area), and many of them have excellent insurance benefits.  Plus there are a lot of excellent hospitals, etc., in the area.  I know you have a lot on your plate right now, but if you're interested I can put you in contact with some recruiters I deal with out here.  Just send me a Private Message if that would interest you.
