    You've got double slashes in your path.

    set @OutputFileName = 'C:\\tracefiles\SQL_Performance' + '_' + convert(varchar(20), getdate(),112) + Replace(convert(varchar(20), getdate(),108),':','')

    The first error is 'could not create trace file'.
