i would simply handle each field with the ISNULL FUNCTION instead:

SET fullname= RTRIM(LTRIM(ISNULL(A.lastname,'')))
+ ', '
+ RTRIM(LTRIM(ISNULL(A.firstname,'')))
+ ' '
+ RTRIM(LTRIM(ISNULL(A.middlename,'')))
FROM B INNER JOIN
ON A.sysID=B.sysID
