    What are criteria to define covering index and how we create it?

    Please provide some details for this question.

    It has to cover the query, and it's created the same way you create any index. I guess I'm not clear on what you're asking here. The only difference between a covering index and a non-covering index is the match-up between columns in the query and columns in the index being complete or incomplete.
