Well, the question of privacy in an LLM is a trivially simple one - there is none.
Guardrails are a lame pretence at securing the LLM, the data can leak, you have no control over where the data is held, nor control over how it's used or who has access to it, and nor do you have any view whatsoever over any aspect of how the information has been used or disseminated.
So, if you work in healthcare, social services, banking, or a myriad other areas where integrity and confidentiality are an absolute requirement, use a cloud LLM to do your patient/client correspondence, and you've immediately breached their confidentiality, and also any requirements such as GDPR, CPRA or any other legislation that has been put in place to (try to) protect people's right to any privacy. Still, who cares when it's been on the cover of Business Weekly - hey, it MUST be the Holy Grail of profitability and efficiency then.
