The volume on device '[path]' is not part of a multiple family
media set. BACKUP WITH FORMAT can be used to form a new
media set. RESTORE DATABASE is terminating abnormally.

What happens if you do Restore HeaderOnly From [backup device]?

Hmm, looks like you have one of the parts of a striped backup. It commonly happens when a backup is taken through management studio and there are two (or more) destinations listed in the box. People often think that the backup goes to whichever is selected, but it's actually stripped across all the destinations listed. If that happens, you need all of the files to restore. If you only have one, you only have part of a backup.
Where did this backup file come from?
