Does anyone know of any free or commercial level tools that can be added to out deployment management process that that manages the deployment of data patches (schema, operational, reference) to multiple databases in multiple environments? A huge added bonus would be the ability to wrap multiple sql patches into a single patch for deployment with transactional support and reporting support (success, granular failure)

While I love VS for DB's and I'm even presenting a session at PASS on how to use it for multi-environment deployments, I don't think it will do what you're looking for. It might come sort of close though, so you should check it out. I'm not aware of any product that's going to do everything that you outlined.
