We ran sql diag in prod system at the same time we saw there was higher response time for some transactions.
How does running the sqlpass diag impact the sql server performance? How it leads to blocking?

It's a monitoring tool. Like all monitoring tools, it will cause additional overhead on your system. I would only turn it on or run it on a short-term, as-needed basis.
