What's in it is easy.
Query sys.partitions join to sys.allocation_units to sys.data_spaces (you can use object_name on the object id from sys.partitions)
Who created it may not be possible to discover. If you don't have any custom auditing, check the default trace and see if it's there. If not then there will be no record of who created it.
