How do I create a scroll down box with check boxes one below the other?

    I am trying to create 10 rows each having three text boxes. Please help me to create this design. I am not able to do it and the check boxes need to be aligned one below the other.

    I don't know what you are using to develop your interface but you would not do this in SQL Server.

    Sorry for not mentioning this. I am trying to create the ASP.NET page and which is connected to the sql server. SO I am doing this in ASP.NET
