We are planning to remotely monitor about 25 SQL servers. I would appreciate if someone would explain or point me to a source where I could find a description where are counters for Memory - Pages/sec, Network Interface - Bytes total/sec, Physical Disk - Disk Transfers/sec, and Processor - % Processor Time being maintained on target machines? Are these values calculated continuously and we just read them or are we causing an additional load on the target machine when we remotely monitor?

I have also a similar environment, RMS(remote management services).But we do with some third part tool that is Hp open view and defined some matrices like you mentioned and that will alert you once threshhold has crossed.
You can use sql trace in QA to get the details of the mentioned metrices and save it in a table. And define alerts in sql when threshhold value crosses.
