Error 18456, Severity 14, State 16

  • Hi All,

    I just observer there are two logins attempt to connect one of my SQL Server instance.

    In the SQL Server logs, not much information about the error, only mention the two error below:

    1. Login failed for user 'xx\xxxx'. [CLIENT: xx.xx.xx.xxx]

    2. Error: 18456, Severity: 14, State: 16.

    I am aware that the error could be due to the default database, I have validated and ensured that the login (service account) has a default database assigned to it and the database doesn't has auto-close configured.

    The error persist even though it configured with:

    - granted with SA

    - granted sysdba for server role

    - added into builtin\administrator group

  • Is the default database online?

    Does the state of the database change at any point during the day when the users could be accessing the DB? Daily refresh or something like that?

    The user has SysAdmin rights?

    The default database is "master". So, I don't think there's any changes on the database.

    Would it be possible for us to track which databases the login try to connect?

  • Can run a profile trace for login failed events to see what its trying to do.

  • Database mirroring in use ?
