    can you show us all the indexes on w_entity_document_test_new and the actual execution plan?

    did your covering index include all teh columns in the query?

    ie

    CREATE INDEX IX_tezt

    ON w_entity_document_test_new (ENTITY_ID,

    ENTITY_TYP_CD,

    DOC_TYP_CD,

    STS_CD)

    INCLUDE( NM,

    FORMAT,

    INTEGRITY,

    TEXT,

    PAR_ID,

    UPDTSEQ,

    CONF )
