I am guessing the login once existed and you set it to remember the username and password? If so you need to remove the following file and then restart SSMS and it should have got rid of the username.

C:\Users\%username%\AppData\Roaming\Microsoft\Microsoft SQL Server\100\Tools\Shell\SqlStudio.bin
