Have to go with failed backups since those end up getting us called back to work. Leaving the wife with the kids might be the scariest of all!

I'm just horribly disappointed that Godzilla did so poorly at the polls. I really thought he would have a shot at it...
Clearly not nearly enough people recall the SQL Server 6.5 sp5/sp5a debacle. You would have a heck of a lot more fear of the SP's when they come out.

After an encounter with auditors that resulting in the entire DBA team receiving verbal warnings and been threatened with dismissal if we ever allowed an audit to fail again, I shudder when anyone says the phrase "Sarbannes/Oxley"

And the 2000 SP3/SP3a and the 2005 SP2, SP2 v2, SP2 v3 messups.

For me it is Failed Backups, definitely. they are so much more common than most IT professionals suspect.
Developers: well, I am one as well as a DBA, so no worries there.
SOX & auditors never bothered me much. I’ve been dealing with MIS audits for 30 years and for some reason I have always gotten along great with the auditors. I find that they are a great way to convince my customers of the importance of certain things.
As for SQL Server Service Packs? Ah, the joys of being a consultant…
Now Windows Service Packs, they scare the heck out of me. I get hosed by them all the time!
