One thing I like to see in addition to the outline in the article is a name and date - so I have a point of reference when looking at code and can easily interact with the developer who originally wrote it.
To me it's my craft - like a painter or carpenter I put my name in there - I am proud of what I have written and in case someone wants to talk about it, be my guest. It helps during daily interaction between dba and developer.
