SQL Server 2014 quorum keeps on getting disconnected from the SQL instances keeps on failing over

    In my client location i have 4 DB nodes and three instances. One Node DB2 having problem it always shows quorum is last that why temporarily we removed that node from cluster owner group. In this regard your expert advise is highly beneficial.

    Windows Server 2012 Standard and SQL Server 2014 Enterprise Edition

    ERROR
    The Cluster service is shutting down because quorum was lost. This could be due to the loss of network connectivity between some or all nodes in the cluster, or a failover of the witness disk.
    Run the Validate a Configuration wizard to check your network configuration. If the condition persists, check for hardware or software errors related to the network adapter. Also check for failures in any other network components to which the node is connected such as hubs, switches, or bridges.

    The cluster Resource Hosting Subsystem (RHS) process was terminated and will be restarted. This is typically associated with cluster health detection and recovery of a resource. Refer to the System event log to determine which resource and resource DLL is causing the issue.

    The Cluster Service service entered the stopped state.

    The Cluster Service service terminated with the following service-specific error:
    A quorum of cluster nodes was not present to form a cluster.

    The Cluster Service service terminated unexpectedly. It has done this 1 time(s). The following corrective action will be taken in 60000 milliseconds: Restart the service.

  • Based on what I see it seems for what ever reason the Quorum drive is losing connectivity in your node.. to avoid this, I have been using file witness quorum on my fileshare server that is outside the node.
    try file share witness  as a  quorum.

    What quorum type have you configured?
    Are all nodes same site or cross site?
    How many network cards and networks does each node have?
