I am so confused I have to normalise these form 1 and 2 together, 2 and 4 together and the rest separately into 3nf. Can somebody help. On the first 2 forms I also have to include the branch, managed by and telephone.these are the forms:
rather than giving us details of the "forms"....are you able to provide the details of the underlying data tables and queries that are being used to populate your forms ?

I just wrote the data As an example. Do you mean staff no: etc...?

appreciate this is in "Design Ideas and Qs"...but have you created any SQL tables yet?
...or we still in design thought stage?

I don't suppose you still have the original question? Anyone else searching for similar help can benefit from the question and answer. Editing it away makes it more difficult for people.

Yeah this is my first time dealing with SQL, thank you for your support, it's good to know that there are people who help as much as they can.
Is it the problem that this is your first time dealing with SQL (as in SQL Server) or is it that this is your first time attempting to do data modeling?
if the issue is the RDBMS then check Books Online, it comes with SQL Server for free.
If the issue is data modeling and you are required to go 3NF please check http://pkirs.utep.edu/cis4365/Tutorials/Database%20Design/Normalization/tutorial%206.00400.htm for a starting point.
