Question:
Sqlserver 2008 R2 Mirroring failed to failover when the primary server got stacked. Environment: 3 virtual servers: Primary, secondary and witness No errors were found in the event log , in the sqlserver logs and in the application.

There are many questions as What type of configuration was it high safety mode or high performance mode?
If this is set with high performance mode with winthness server then there is risk associate with it
When a witness is set in a high-performance mode session and If the principal server is lost, forcing service to the mirror server requires that the mirror server be connected to the witness.
If the mirror server is lost, the principal server must be connected to the witness. Otherwise, the principal server takes its database offline until either the witness or mirror server rejoins the session.
Hence please check the configuration.
